Haha, I changed my header yesterday. Had this image lying around, but I didn't feel it was as epic as the No Hero one (I really liked the scene in that one). Still, twelve knights lined up together looks pretty impressive. XD This is drawn by the same artist (Wu Ling) as the person who drew the No Hero pic. I like her art a lot. :D
Stone and Ice are indeed fairly tall (38th generation Stone and Ice fit the heights!). But Eaaaaarth. Earth is adorable. That shield is as big as him. Yup, Metal is the one next to Judgment. He looks somewhat narcissistic there, haha.
You can customize the header and background with any account level, but it usually requires some CSS. You go to Journal -> Journal Style (pick what kind of layout you want, such as flexible squares, smooth sailing, etc) -> Customize Your Theme (on the right under more options). On the left once you're at the customizing your theme page, there's an option for custom CSS where you can use CSS to specify how you want your entire layout to look like. If you want your own header image, the custom CSS page is usually where you specify it!
The only part of my layout that needs a paid account is the buttons for leaving a comment. I had to set that up while I still had a paid account (I don't have a paid account anymore). It requires setting up a custom theme layer. I'm not too sure how it's done since someone else helped me with that one, haha. My custom theme layer also made it so that my tags display to the right of my mood pic (the default setting isn't like that).

The other mini icons though, (like the little cat I have next to usernames) can be set up through CSS. This is a generator for the code to use with that!

If you want to set up a mood theme, plus accounts can do that too. :) That is also under customize journal style. I have a few custom mood themes I made, like these: LSK, La Corda d'Oro, Ookiku Furikabutte, Prince of Tennis, Fullmetal Alchemist.
